How to play Happy Birthday on acoustic guitar?

My mothers is turning 39 ,August 28th. I wanted to do something special for her, and seeing as how I have a guitar, I wanted to play happy birthday for her. I have no idea how to actually play guitar, I know the cords, but that's it. Any easy ways? :)! If you can find a video, or website, that would be so awesome. I have two weeks, so I have time! Thanks everybody.

5. I had trouble finding the chord melody arrangement for happy birthday, you can try to google it and you may have better luck than I did. allguitartabs.com has the individual note style.
